Can't get it to work

So, I am trying to create an audition sign up for a theatre show. I have created the location (only one), the staff (only one), I have chosen the two days for auditions in the “Work Schedule” area, I have also chosen all other dates around the two days available for auditions as “days off”.

When I place the form shortcut on the webpage, it doesn’t give me a calendar or anything, and the stuff it does give me I am not thrilled about.

Here is what I would ultimately like to do:

An actor goes to the page to “sign up” for an audition. They get the option of the two dates and choice to 15 minute window for their audition, they chose their time, and they enter their contact information.

I am usually really good at this stuff, but I can’t get it to do what I want. I purchased the
